DIABETES SELF-MANAGEMENT PROGRAMME
The HYVE Diabetes Project is a ‘Point-of-care, self-management approach to empower people to take control of the condition.
The project will be launched by The Mayor and Mayoress of Kirklees Cllr Paul Kane on the 12 May 2016 at The St John’s Resource Centre, 29 St John’s Road Birkby, Huddersfield. HD1 5DX
The programme, designed by Eagle Eye Enterprises Ltd, in collaboration with the Honeyzz Diabetes Support Group, and funded by the BIG Lottery “Awards for All” programme will enable people with diagnosed and un-diagnosed diabetes to have the vital elements associated with the condition, (blood pressure, blood sugar, cholesterol, weight, foot care) checked and monitored. The programme will also empower sufferers to learn more about living comfortably with the condition.
From the 18th May2016, the project will be ‘open for business’ each Wednesday 10 am to 1pm.

This community based initiative came out of a community consultation “Improving Diabetes Care”, which sought to augment the medical approach to diabetes care.
The Director of the HYVE project says “With this self-management facility in place, people can now become active participants, rather than passive recipients in their care”
Eagle Eye Enterprise Ltd is a local managing and enabling social enterprise, which has been working alongside small organisations and SME’s over the past 6 years in order to accelerate their development and growth.
